What is this icon?

I'm on version 6.503. Could someone tell me what these icons are (see images below) and how to get rid of them or fix them? They appeared when I mistakenly hit command D (duplicate) and now anything with that icon doesn't work - have to completely redo that floating menu.

command d is disable, not duplicate. So this means the selected items are disabled. You can reenable them by pressing cmd d again
