How to toggle CONTEXT menu items?

I would like to use one BTT gesture to toggle context menu items, for example in Bear, if I click on a tag, I can right click → "pin" or "unpin" the tag.

I don't use Bear so I am not sure, but one trigger (gesture), two actions, that should work with a cycle action. Have you already tried this?

If Bear is a native macOS app, you should be able to use the "trigger context menu item" action with this command (make sure the lower / uppercase is correct):

pin||unpin
