One thing that would be awesome to have "hide left of menu bar icon set" and not only left of BTT Icon. So I could move the custom icon somewhere and hide everything left of it.
Also, I am confused when it says "left of BTT icon" but it hides the BTT icon and everything left of it ...
have a look at the preset shared in the first post. You can define custom items and move them around however you want (best used with BTT >= 4.575)
Guess me not having the latest version (I was on 4.3something) was the reason it didn't work. I will look into this again.
That is a terrific reply - much appreciated. There is so much more BTT can do that I had not realized.
I'm also currently playing around with the template from the first post. It seems to work, but after double click on the menue bar to show the hidden items there always a popup comes up "Apple Script starten...." which needs to be confirmed. I guess there is somewhere a checkbox to skip those confirmations!?
That is not coming from the preset, it doesnât use Apple Script (it does, but run directly from BTT withou confirmation). Do you maybe have some othert app that has assigned an apple script to menubar double click? Does that still happen if you quit BTT?
It definetely from BTT and as soon as I click on that the hidden items will be shown. By closing BTT nothings happen with the menue bar.

I think I understand what's going on here. On the MacBook Air I have enough apps with menubar icons installed that several are getting pushed left and hidden behind the notch. One app just told me that this is what is happening for its menubar icon. So I think the BTT menubar icon is somewhere behind the notch. (Of course, there's no problem on my Mac mini and Studio Display because there is no notch.)
you have two âmenĂźleiste doppelklickenâ configured there. Maybe you already had one of them before you imported the preset.
the Apple script one is the correct one from the preset. Delete the other (or at least only keep one of them)

Edit: But I'm also facing the issue with "CMD" and "option" key.
By using the preset and items are hidden via custom menue item, they will be shown again by pressing CMD. That will not happen by using the function to hide all items left of BTT icon.
it seems that is something from the preset shared by @JessePeden , did you also import that? (Bartender controversy, tutorial on how to manage menubar status items via BTT - #38 by carycrusiau)
No, I imported only yours.
Could you check whether setting this to "Show always" fixes it for you? (If so I'll reexport the preset, maybe I made a mistake with one of the exports)
no, does not change anything in the behaviour.
Edit: Now I disabled and enabled one of the custom menue items and are facing the same crazy stuff as above mentioned. By default the menue item is hidden and by holding "CMD" it will shown. 
I think I have an idea about what could cause this, Iâll be back on my computer in an hour to check!

There was indeed a bug that could cause this. It should now be fixed in v4.576!

Now it's working fine for me.
Finally two things from my side for your long menue bar feature list:

Custom menue items from the template should keep visible by hiding the items left to it, so that with another click on that menue item the hidden items can be restored.

Position of moved menue will not be stored after a restart or closing the application so that in some situation a reordering is required.
But in general BTT is on a good way to replace the basic functionalities of Bartender. Thanks a lot for that. 
I now have a new problem. On my MacBook Air, Settings in BTT is greyed out. Everything is working fine in all regard on my Mac mini. Both OSs are the same, current version. In fact, BTT was working fine yesterday when I was working on the Bartender/menu items issue. But today I have no access to BTT settings because the menu is greyed out.
