Hey,
i tried to use my key mouseclicks in diablo 3. But it wouldnt recognize it at all.
I use it for other games and there it works. its just this one.

Have you tried to debug it using the "Emulate Selected App as Active" Feature?
How does your trigger look like?
Most likely the game intentionally ignores programatically generated mouse events, e.g. to prevent cheating.
I don't have much experience with games on macOS, but I guess there is not much you can do.
