Bug when adding another action along with context Menu as a custom menubar icon

Describe the bug
Added custom Menubar icon, context menu as action. Works well until adding another action under the same automation, it deletes the menu.
Deleting added action is not an option.

Workaround: I use CMD+Z repeatedly to restore everything back to normal.
Is it impossible to add action next to context menu?

Affected input device (e.g. MacBook Trackpad, Magic Mouse/Trackpad, Touch Bar, etc.):
Macbook pro

But when I add another action:

Becomes weird looking and it adds some + buttons,...

I use CMD+Z repeatedly to restore everything back to normal.
