I had this same issue starting a few years ago.

There is a line of text you can enter into Terminal that will fix this.

unfortunately I never figured out what causes this. It seems to happen on (very few) systems, since macOS 10.12. I assume something on the system is trying to launch BTT again while it is already running, which will cause the prefs to show. The only workaround is this: Configuration window appears out of the blue - Bug Reports - BetterTouchTool Community (folivora.ai)

I have received two logs ( thank you!) and they show that something on the system tries to launch BTT while it is already running. I have no idea what that could be (and unfortunately I don't know of any way to find the calling process). But for now I have added a hidden setting that disables showing the preferences window when that happens.
