Add New Item -- cmd-+ -- or pasting -- need help understanding varied behaviors

OK, on a good number of my triggers, when I press the + sign in the actions column (or cmd-+) or at the bottom of all the actions, an item gets added immediately underneath the currently-selected item:

But on some of my (maybe older?) triggers, it decides to add the item on the bottom of the items:

Is this something I've asked it to do, unwittingly?

Similarly, very often, if I copy an item that I want to paste immediately underneath a specific action, I need to choose the one above the specific action -- I find this a bit strange compared to all other pasting experiences across applications. Is this also something I've asked it to do, unwittingly?

You'll see me select and copy sh-cmd-H action
Then I select ^G action
Then I paste
BTT deposits sh-cmd-H action under the Type Text action instead of the ^G action

this is indeed related to action sequences that have been created > 1 year ago.
Back then I improved the data model to allow better insertion behavior. However in order to not break anything I didn’t convert existing action sequences to the new format.
